This raffle is open to individuals who complete an Adult Cleaning appointment at any The Chicago Dental Studio location between October 1, 2025 and October 31, 2025 (“Promotion Period”).
Eligible participants include both returning patients and new patients.
Limit one (1) entry per patient during the Promotion Period.
To qualify, an Adult Cleaning appointment must be scheduled and completed during the Promotion Period.
One (1) winner will receive a Louis Vuitton Speedy Bandoulière 20 handbag.
Approximate retail value: $2,770.00.
The prize is non-transferable, non-exchangeable, and cannot be redeemed for cash or substituted for any other item.
The winner will be selected in a random drawing conducted after the conclusion of the Promotion Period.
The odds of winning depend on the total number of eligible entries received.
The winner will be notified using the contact information provided at the time of entry. If the winner does not respond within 48 hours, another winner may be selected.
By participating, entrants agree to be bound by these Terms and Conditions.
The Chicago Dental Studio reserves the right to disqualify any entrant who tampers with the entry process or violates these Terms and Conditions.
The Chicago Dental Studio reserves the right to cancel, suspend, or modify the raffle in the event that it is not capable of running as planned due to technical failures, fraud, or other causes beyond its control.
By accepting the prize, the winner consents to the use of their name, likeness, and general demographic information (such as city and state of residence) for promotional purposes in any media without additional compensation, unless prohibited by law.
In compliance with the Health Insurance Portability and Accountability Act (HIPAA), The Chicago Dental Studio will not disclose any protected health information without the patient’s prior written authorization.
If desired and with written authorization, the winner may allow The Chicago Dental Studio to use their testimonial, before-and-after photos, or treatment story for promotional purposes, including but not limited to the practice’s website, social media platforms, and marketing materials. Consent for such use is voluntary and not a condition of receiving the prize.
The Chicago Dental Studio is not responsible for any incorrect or inaccurate information supplied by entrants or for any technical failure of equipment, systems, or networks.
By entering, participants release The Chicago Dental Studio from any liability arising out of participation in the raffle or acceptance, use, or misuse of the prize.
