Official Giveaway Rules Of The Chicago Dental Studio 2025 Invisalign Giveaway. No Purchase Necessary To Enter. A Purchase Will Not Increase Your Chance Of Winning. The Giveaway Is Offered Only In The United States Who Meet The Stated Eligibility Requirements. Winner Must Be Able To Receive Treatment At A Designated The Chicago Dental Studio Office.

ELIGIBILITY:

The Giveaway is open to residents of the United States of America and age 18 or older who are physically able to attend treatment at a designated studio owned and operated by The Chicago Dental Studio located in Chicago, IL.

Employees and directors of Sponsor, its affiliated companies and companies associated with the implementation, administration and fulfillment of the Givaway and individuals employed by Sponsor or contracted and their immediate family (parents, siblings and children and their spouses) and members of their households, whether related or not, of such individuals are not eligible.

This Giveaway is void where prohibited. The Giveaway Winner must sign (1) an Affidavit of Eligibility, (2) a Release of Sponsor and its employees, officers, independent contractors and affiliates from any and all liability, claims, demands, and causes of action for personal injury and/or damage, theft, or loss suffered in connection with this promotion or the use or acceptance of the Prize or any portion thereof to be eligible for the Prize Winner(s); (3) an Agreement to Sponsor’s Terms and Conditions for Invisalign Treatment (4) where permitted, a Publicity Release granting the right to use the names, likenesses, voices, and opinions for advertising and publicity purposes without additional notice or further payment or compensation, including but not limited to the publication of such information of the Winner on any of Sponsor’s websites.

Entry in this drawing constitutes each potential Winner’s agreement to sign such releases. Noncompliance will result in disqualification of the Winner, and an alternate winner may be selected from among all remaining Eligible Entries, in Sponsor’s sole discretion. Any Violation Of These Official Rules By The Giveaway Winner (At Sponsor’s Sole Discretion) Will Result In Such Winner’s Disqualification As Winner Of This Giveaway And All Privileges As Winner Will Be Immediately Terminated. TIMING: The Giveaway will be posted on thechicagodentalstudio.com, starting at or around 12:00AM (CST) on April 1, 2025 and ending at 11:59 PM (CST) on June 30, 2025 (the “Promotion Period”). During the Promotion Period, access the Giveaway entry page at thechicagodentalstudio.com and follow the directions to complete and submit an official entry form. Incomplete or inaccurate entry forms and failure to follow @chicagodentalstudio on Instagram will be considered ineligible and void. All entries must be submitted and received by 11:59 p.m. CST on June 30, 2025, Limit of one entry per person. If a person submits more than one entry, only the first entry will be accepted. In the event of a dispute as to any online entry, the authorized account holder and/or user of the email address used to enter will be deemed to be the entrant (“Entrant”).

The “authorized account holder” is the natural person assigned an email address by an Internet access provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the submitted address. Entrants may be required to show proof of being the authorized account holder. All entries become the property of Sponsor. Information submitted on each entry, including but not limited to personally identifiable information about Entrant, such as Entrant’s name, treatment details, e-mail address or mailing address may be used by Sponsor. Sponsor may contact Entrant occasionally to inform Entrant of new services or promotional material.

Sponsor may use the information submitted on each entry for marketing purposes and market research or to help improve its services. No cash or other substitution of prize is available. Prize may not be transferred.

WINNER SELECTION/NOTIFICATION:

The Giveaway winner will be selected in a random drawing to be conducted by Sponsor on or around July 1, 2025. A representative of Sponsor will contact a potential Winner at the email address provided in the potential Winner’s entry.
Sponsor will make no more than two (2) attempts to email the potential winner over a forty-eight (48) hour period. Upon notification, the potential winner must confirm his or her identity and eligibility, by signing and returning an Affidavit of Eligibility within seventy-two (72) hours of initial contact to Sponsor.

If an alternate Winner is chosen, he/she will be selected randomly from the remaining eligible entries. All prizes will be rewarded unless declined by Winner. Odds of winning will be determined by the total number of eligible entries received. PRIZE: Complete Invisalign Treatment as diagnosed and proposed by The Chicago Dental Studio.

The Prize is subject to eligibility, availability of Sponsor, all other terms herein and Sponsor’s Terms and Conditions. AGREEMENT TO OFFICIAL RULES: By participating in this Giveaway, Entrants agree to be bound by these Official Rules and by the decisions of Sponsor which are final in all matters relating to this Giveaway.

GENERAL CONDITIONS:

By participating, Entrants agree (a) to release and indemnify and hold harmless Sponsor, its parents and affiliated companies and their respective directors, officers, employees and agents from and against any and all liability or claims resulting in whole or in part, directly or indirectly, from their participation in the Giveaway and the acceptance and use of the prize as further set forth below; (b) if the winner, to the use of name and photographs and video in advertising or publicity relating to the Giveaway and/or the Sponsor in all media.

LIMITATIONS OF LIABILITY:

Sponsor is not responsible for any incorrect or inaccurate information, whether caused by website users or by any of the equipment or programming associated with or utilized in the Giveaway or by any technical or human error which may occur. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or unauthorized access to, or alteration of information submitted; nor is it responsible for any problem or technical malfunction of any equipment or system, or for any late, lost or misdelivered mail. If for any reason the Giveaway cannot run as planned, including for any cause which corrupts or affects the administration, security, fairness, integrity or proper conduct of the Giveaway, Sponsor reserves the right at its sole discretion to cancel, terminate or modify the Giveaway. Notice of any such action will be posted at www.thechicagodentalstudio.com. If the Giveaway is terminated or canceled, Sponsor shall select the winner in a random drawing from among entries legitimately received prior to the event causing such termination or cancellation.

Release: By Entering This Giveaway, Entrant Agrees To Release And Hold Harmless The Chicago Dental Studio And Its Officers, Directors, Shareholders, Employees, Agents, Independent Contractors And Affiliates From And Against Any Claim Or Cause Of Action Arising Out Of Participation In The 2025 Invisalign Giveaway Or Receipt Or Use Of Any Prize In Any Respect.
