CDS Subscription Information 

Welcome to the Chicago Dental Studio Subscription program! Before you enroll, we kindly ask you to take a moment to carefully review our terms and conditions as they contain essential details about your subscription purchase, including renewal and cancellation procedures. 

Here’s what you need to know:

  • Managing Your Subscription: All CDS subscriptions are easily managed through the patient subscription portal, which you’ll have access to upon enrollment. 
  • Renewals: CDS subscriptions are continuous and automatically renew at the end of each period, so you don’t have to worry about interruptions in service. If you ever need to pause or cancel, you can do so manually via your patient subscription portal. 
  • Payment: By enrolling in a subscription, you’re authorizing us to charge the payment method linked to your patient subscription portal when your subscription renews. If you need to update your payment method or make other changes, you can easily do so through your portal at any time.

Terms + Conditions  

  • We require a minimum of 2 billing cycles before we allow cancellation of your subscription. 
  • Subscription cancelation must be submitted at least 10 days prior to your next billing cycle to cancel your next scheduled order. 
  • Cancelling a subscription does not provide you a refund for payments already processed,  nor does it stop a pending charge.  
  • Your cancellation will go into effect the following billing cycle.  
  • All subscription orders are non-refundable and non-returnable

CDS Subscriptions are set for a minimum of two (2) deliveries before you are able to cancel.  Once your 2nd cycle minimum has been completed, you are free to cancel or pause your subscription at any time via your patient subscription portal.  

All fees related to your CDS subscription will be charged to the payment method type provided on the exact payment date, as indicated on your patient subscription portal page. The duration of your subscription period will depend on the subscription type you selected when you signed up, and you can find the specific details clearly outlined on your patient subscription portal.  

We value your subscription and want to keep you informed about how it works. While we may occasionally need to modify our subscription plans or adjust pricing, we promise to do this only when necessary and in line with our commitment to providing quality service. Should there be any changes to your subscription plan or pricing, we’ll make sure to notify you in advance. Rest assured, these changes would only take effect after we’ve communicated them to you, as we want to keep you well informed about your subscription.
